All the extra buttons work on macOS. The top two small buttons work for changing DPI & thumb buttons work on Chrome for navigating webpages forward or backward (didn't work on firefox). I think the buttons worked after I installed synapse 3 on Windows 10 through Bootcamp and configured it.
There is no Synapse 3 on macOS, which is a big downer for me. I can still store profiles using Synapse on Windows though.

To preface this, just wanted to say that I'm by no means bragging or boasting, just giving my 2 cents as someone who has played at one of the highest levels of CS. I played ESEA-Premier/MDL as well as CEVO P. My first ever gaming mouse was the deathadder, loved the original shape and it's been my main mouse ever since.
I have tried just about as many other alternatives as I can including the Zowie EC2 A, Zowie EC1, Finalmouse, Steelseries Rival, Logitech G403, xtrfy m4, etc. I always somehow going back to the Deathadder because of it's shape, I guess I've just gotten so acclimated to it. Since 2009 I've gone through about 4 of them, some have been due to wear from the left trigger button, and one because the side buttons stopped working. Though one of the biggest issues for ME has ALWAYS been the cheap and horrendous side grips which always fall off after 6-8 months. Hot glue, gorilla glue, nothing would make it stay. So after countless iterations of the deathadder, finally the v2 has addressed this issue and more.
Major changes that are small but absolutely game changing:
-Side grips are now integrated into the left and right side of the mouse, they are a physical part of the shell and therefore cannot fall off, this makes cleaning easier
-20k dpi is absurd but the sensor is improved and has a higher IPS than the elite
-mouse skates are similar to the elite and are now two much larger pads that cover the top and bottom of the mouse, much better than the previous tiny skates they had prior
-the shape on the right mouse button has been ever so slightly adjusted, it doesn't flare out as much and feels more comfortable to grip
-weight has been reduced to 82g
-the DPI buttons, which I don't use, are more spaced out and therefore avoids accidental hitting
-Mouse switches are optical, they're not as loud and feel just as responsive as before
All in all, this is definitely the best iteration of the deathadder they've put out. It improves on things that have been a hinderance on every model since 2009.
