It comprises a sucker on a short handle. The sucker attaches to the end of the bulb, the handle twists through 90 degrees to sort out the suction and then the bulb proved very easy to twist out of the fitting. Whilst I held a hand underneath as a precaution against the bulb falling after removal, I needn't have worried as, once attached to the sucker, it was stuck until I twisted the handle back through 90 degrees to release it.
A certain law dictates (with a fair degree of logic in this case) that a bulb will usually blow at night when it is difficult to replace in the dark. I found it was the simplest task to replace the bulb by repeating the process in reverse (apply sucker to bulb, twist handle then insert in the fitting and remove tool once bulb tightened) - the handle enabled the new bulb to be inserted when the rest of the lights were on; I could see what I was doing and, when the new bulb lit up, the handle ensured I did not get burnt hands when the bulb got hot very quickly....
All in all, this is a simple and economic solution to what had become a bit of a pain. If only everything in life could be resolved so easily!
